CUPA File Review Request

Thank you for contacting Alameda County Department of Environmental Health (ACDEH) Certified Unified Program Agency (CUPA) File Review Request:

Consider the following in making your request:

  1. Make sure you have a Physical Site Address. (i.e. 1234 Burch Lane Oakland, CA 94603)
  2. Only having directional information will limit the ability to locate the file you are looking for. (i.e. The location is by or on Burch Lane in Oakland CA)
  3. None of the following are needed, and they do not assist in locating your file.
    • APN
    • Parcel Number
    • Longitude and Latitude information

A physical site address is needed to administer a search.

**Please note that ACDEH only has files for sites within its jurisdiction. Many cities within Alameda County are not part of ACDEH CUPA jurisdiction.

In those cases we advise that you try contacting that cities Fire or Planning Departments for any records.
