Applications for a license or permit to collect or transport garbage, refuse garbage, or commercial garbage in the unincorporated area must be made in writing and signed by the applicant and must show the following:

    A.    The name, address and telephone number of the applicant;

    B.    The type, kind and make of each truck or vehicle to be used by applicant in the collection and transportation of garbage, refuse garbage or commercial garbage;

    C.    Whether such truck or vehicle is so constructed as to prevent leakage, dripping or overflow therefrom;

    D.    The type and kind of cover used for covering its contents;

    E.    Place or places of disposal;

    F.    The method, manner and frequency of cleaning such equipment;

    G.    A statement as to mechanical condition of such equipment;

    H.    A statement that applicant will not assign or transfer such license or permit, and that applicant agrees to comply with all the requirements of this chapter now in force or as hereafter amended, and with the conditions of the permit issued for each truck or vehicle engaged in such collection and transportation.
