Family Medical Leaves

Family Medical Leaves (Family Medical Leave Act/California Family Rights Act) provides eligible employees with up to 12 weeks of job-protected unpaid leave in a 12-month period for the following qualifying events; (1) serious health condition, (2) need to care for a family member with a serious health condition, (3) disability due to pregnancy, child birth, or related medical condition, and/or (4) need to bond/care for a new born adoptive/foster child.

Additionally, the Family Medical Leave Act provides special provisions for military family leave.

California's Pregnancy Disability Leave provides employees up to 4 months of job protected leave due to pregnancy, childbirth, and/or related medical conditions. There are no eligibility requirements for Pregnancy Disability Leave.

To Request a Leave

The forms to be completed will depend on your FML request. Please complete the required forms as outlined below and submit to your Agency/Department Human Resources Department.

If you are not eligible for FMLA and need to apply for Pregnancy Disability Leave, please contact your immediate supervisor or your Agency/Department Human Resources Department.
