Online requests for unrestricted certified copies of certificates must submit a statement declaring that you are authorized to receive such a copy.

If the order will be picked up from our office:
The person who submitted the request must present official government identification and sign the statement under penalty of perjury upon arrival. If supporting documents are required, you must submit those documents with the unsigned authorization statement to our office before the order can be processed. Failure to do so may result in cancellation of your order. Your order will NOT be processed until required documents are received.

If the request is to be delivered via mail or express delivery:
The statement must be notarized (signed in presence of notary public) and submitted to our office before the order can be processed. If supporting documents are required, they must be submitted with the notarized statement. Failure to do so may result in cancellation of your order. Your order will NOT be processed until statement is received.
After submitting your online order, a pre-filled Authorization Statement will be prepared for each certificate requested that requires it and available for printing from the Confirmation page and email.

Note: International requests for unrestricted copies must be notarized at a United States Embassy.
