Permanent Supportive Housing

Permanent Supportive Housing: Permanent affordable housing accompanied by supportive services to assist homeless persons with disabilities to live independently.

Shelter Plus Care Programs:

HCD's Shelter Plus Care (S+C) programs provide on-going rental assistance accompanied by community provided support services to homeless families and individuals disabled by serious mental illness, chronic alcohol and drug problems, and/or HIV and related disorders. Housing components include tenant-based, sponsor-based, and project-based assistance. The program serves over 566 households countywide. For more information or referral, call: 2-1-1, Eden I&R.

  • S+C Tenant-Based Rental Assistance (TRA): Once accepted into the program, participants work with designated community based agencies to locate a rental unit in the community. The rental assistance is provided to the landlord and the tenant pays 30% of their income towards rent. The tenant may change housing sites and maintain their rental assistance. HCD currently assists 410 participants and their families in the TRA component, including 85 in programs linked with three Behavioral Services Mental Health Service Act Full Service Partnerships for chronically homeless adults with serious mental illness.
  • S+C Sponsor-based Rental Assistance (SRA): Provides housing assistance at designated housing sites owned or managed by nonprofit Sponsors. HCD's SRA component is comprised of 17 sponsors and currently assists 125 participants, including 59 units at the Harrison Apartments in downtown Oakland and other sites in various cities.
  • S+C Project-based Rental Assistance (PRA): The Alameda Point Shelter Plus Care PRA Program provides permanent housing and supportive services to 23 formerly homeless individuals with disabilities and their families: 14 at Alameda Point and 9 at the Lorenzo Creek Apartments in Castro Valley.

Supportive Housing Program Projects (SHP) Permanent Housing Program

HCD's SHP programs provide affordable housing operating subsidies and support services for homeless individuals and families.

  • Lorenzo Creek: Nine units of permanent supportive housing for homeless people disabled by serious mental illness, chronic alcohol and drug problems, or AIDS and Related Disorders, and their families in an accessible multifamily housing complex constructed using principles of universal design in Castro Valley. Lorenzo Creek residents receive rental subsidies through S+C and supportive services both on and off site through SHP. For more program information, dial 2-1-1, Eden I&R.
  • Spirit of Hope I: Located at the former Alameda Naval Air Station, Spirit of Hope I provides permanent housing for homeless families with disabilities. Specifically, the project serves large families who were formerly living on the streets or in shelter.
