INFORMATION SYSTEMS ANALYST
Supplemental Questionnaire
As stated in the examination announcement for this position, a
properly completed Supplemental Questionnaire must be submitted
with an application. Failure to submit the Supplemental Questionnaire
will result in disqualification. Applications and Supplemental Questionnaires
must be in the possession of the Human Resource Services Department
by 4:30 p.m. on the Last Day for Filing. Postmarks are not accepted.
The purpose of this questionnaire is for you to identify your qualifications
and experience in job-related areas. A panel will review applications
and Supplemental Questionnaire responses of candidates who meet
the minimum requirements and select the best qualified candidates
who will then continue in the examination process.
Although you may possess the minimum requirements for this examination,
you are not guaranteed advancement in the process. It is critical
that you fill out the Supplemental Questionnaire completely, listing
all education, experience or special training which might demonstrate
your qualifications in the rating areas.

Attach this cover sheet, and put your name at the top of each page.
Your answers should be concise, complete and clear. Grammar, clarity
of expression and legibility will be considered in the evaluation
process. Try to limit your responses to half a page for each question.
Describe your experience in each of the following areas. In so
doing, describe each position in which you performed these duties
separately. Be sure to indicate the name of the employer(s) for
whom you performed these functions, the number of hours worked per
week and the percentage of time you spent performing the duties
described.
1. Conducting systems analysis. Analyzing business functions, work
flow and/or organizational structure. Recommending improvements
to systems or structures to increase productivity or efficiency.
2. Coordinating and implementing new computer systems and programs.
3. Developing new or revising existing programs for user applications.
Developing specifications for obtaining software and hardware for
departmental applications.
4. Training personnel in the use of computer software and hardware.
5. Developing programs and applications for mainframe, personal
computer and network applications.
6. Creating and maintaining systems manuals. Developing and maintaining
procedure manuals.
7. Maintaining overall responsibility for a multi-user data processing
operation, including (but not necessarily limited to) overall system
security, software, hardware and communications maintenance and
troubleshooting.
