Employee's Essential Job Function – EF5

Overview

The EF5 form is intended to describe the employee’s essential job functions. These are identified as those duties that are required to accomplish the purpose of the job for which the employee was hired.

Who completes the EF5?

Generic EF5’s should be stored by the Agency/Department Workers’ Compensation Liaison. On a case by case basis, an outside vendor might be utilized for the purpose of developing a new EF5.

What happens with the EF5?

The completed form will be reviewed by the treating/evaluating physician to determine whether the employee is able to perform the essential job functions and return to his/her job or an alternate position. This is an important document to obtain information on how an employee’s medical condition impacts his/her ability to perform the essential job functions with or without a reasonable accommodation.
